Please carefully read the Photocontest instructions before submitting this form. Photos that do not meet the requirements will NOT be used in the calendar.

IMPORTANT!! Do NOT close the window before you receive the confirmation screen. Questions and problems should be REPORTED TO US IMMEDIATELY. 

All Submissions will include Child's First Name, City, State or Region. City and State are optional. If you decide not to include them, please include a "regional" description for the region (for example: "Deep South", "New England", "Mid-West", etc.).

FEATURE PHOTO ENTRIES: please make sure to also include a TITLE and the photographers name (or nickname)  in "Photograph by".  Review the Photocontest instructions to make sure that you have included a picture with the proper resolution and which meet our requirements.

** Please note that most professional studios own the copy right on photos.  By default, we will not accept studio photos.  However, if you are the photographer, please note this in the comment..
