Embassy requring a new form

Effective September 1, 2006, The US Embassy in Guatemala is requiring families to submit a new form. This form will need to be notarized (but not certified or authenticated) and submitted to the Embassy along with all documents required to get DNA test approval. Correction: I peviously posted that this form should probably be considered part of the dossier. I was incorrect in that and really it would most likely be sent to Guatemala with your POA.

My attorney is currently trying to get authorization and doesn't have this new form.

Who should mail it to the embassy? Should I fill it out and mail it to our attorney or do I send it straight to the embassy since our attorney is currently getting DNA authorization and possibly already gave the rest of the papers to the embassy???

The new rule doesn't take effect until Sept 1 so there is no need for them to submit it. The goal as I understand it is to have a formal way of collecting accurate e-mail addresses so that the Embassy can communicate with you, notify you of pre-approval, etc. There has been a less formal system in place (not sure how it works) for doing this but I guess the embassy has been getting some incorrect e-mail addresses. So by having you send a notarized form, that should alleviate the problem.

once it tkaes effect, the attorney will submit it along with the I600 they provide when they go in to get DNA approval.

I'm glad to see the embassy doing this. During our adoption, they must have gotten our email address from our agency (because we never gave it to them), and it was incorrect. Luckily, the Embassy cc'd our agency on emails and our agency would forward them to us. But, it was frustrating to be at the mercy of our agency to receive information that was intended for us directly.
Although it is yet another form to fill out, I'm glad to see the embassy asking for email addresses in a more formal manner.

Agencies have been supplying this information to the attorney as a part of their submissions of the dossier. The biggest problem has been when changes have taken place and the Embassy has been notified they have not made the changes...... I personally changed and corrected my e-mail with them four times since January and they still send me e-mails to the old address....... Yes, it does not take affect until September but I am updating all my clients who have not had DNA yet at this time, by having them do the document. There is also a document for the agency e-mail, done by the agency director. I sure hope this works well.

this is a great form. I feel like this actually puts the parents in charge of how they receive the info. All we knew was that the embassy would send us an email, we didn't know how they got our email and of course it was incorrect so we didn't get it and then had to get our senators office involved for no real reason.

My question is, my husband and I are just about finished with our homestudy and Dossier and we expect to turn it in around the first of September. Do we fill out this form and supply it with the dossier, because we obviously do not have a referral and don't know when it will be till we get one. It's supposed to be submitted with DNA, so do we get it notarized leaving the child name blank and the attorney fills that part out? Would appreciate any feedback on my question.

As to when to sign/send the new form: it obviously couldn't be completed until the child was identified and of course it could not be notarized before that point. So it's not practical to do it with the dossier since that should ideally be complete before a family accepts a referral. We will be having our clients send it down with the power of attorney, or possibly earlier if the power of attorney takes more than a week to process and get to Guatemala. Otherwise, it could delay getting the case filed with the embassy in order to get permission to do DNA. I am also glad to see the embassy formalize this because the email addresses of the client, the agency and/or the attorney are often incorrect in emails the embassy sends.

For those of you adopting without an agency, the CIS office in Guatemala told me late last week that the form from the adoptive parents only need to be amended to mention that there is no adoption agency. I wanted to check into it because I didn't want the attorney to get turned away at the window because he didn't have the documentation from the agency.

The model/sample posted here has the document on letterhead with a logo. My attorney has heard from some that the letter from the adoptive parents should also appear on that letterhead and then some say that it can be on "plain paper." Does anyone know?

Hi, I got this new form into my adoption agency as soon as I learned about it. I was told by them that these new forms may cause a delay of up to one month! Has anyone else heard this? After reading the posts on this subject, none of the posts others have placed seem to mention any time delay. I am hoping my agency is misinformed. Any thoughts? Thanks.

LB- We got KO in July because this form was missing. We were back in August 1st so I can't see where this is setting anything back. We also filled out the form and sent it immediately when it was available. Our little guy just turned 6 months on August 18th. Went into PGN on June 26th. Anxiously waiting........

Kerrie - Now that you are in PGN how long do you expact it to take until "Gotcha" day? Our little guys just got approved for DNA testing. I am not really getting any time information updates from my agency so I am wondering what it has been like for you all? Thanks. - LB

LB- DNA for us was very quick (3days). Then we entered Family court and were out before we knew it. The difference is whether or not the Birthmother is willing to cooperate or is even around for them to test. We entered PGN on June 26th with one KO in July and back in August 1st. Waiting will be very difficult but we get videos every two months. If your agency is not sharing information it is very hard to deal with. Our agency shares whenever they get new information but it is not very often. We got notified via email about our pre-approval. We were told that we could get out of PGN anywhere from Aug 28th to Sept 27th. We are praying every day that we will get that email or phone call. I understand that it could take a while after you are out of PGN to get the "Pink Slip" to go to for him medical appointment. The waiting will be worth it I know but our little guy will be 7 months old on Sept 18th and I don't want him to be scared when he comes home because the older they get the harder it is for them to adjust.
